For many health systems, behavioral health is no longer confined to a single department or care setting. Patients may encounter behavioral health needs in ambulatory practices, emergency departments, inpatient settings and through their health plan.
In this webinar clip, Tracey Izzard of Sentara Health shares how the organization has shifted from isolated, clinic-based behavioral health services toward a more coordinated, enterprise-wide model.
That approach includes expanding behavioral health training, leveraging telehealth and placing behavioral health navigators across emergency departments, the health plan and provider settings. Sentara has also centralized governance and expanded screening and triage across multiple points of care to create greater consistency across the system.
The goal is to reduce fragmentation and create a more seamless behavioral health experience across care settings, including supporting transitions in care and potentially helping some patients avoid an emergency department visit.
