Cancellation Policy
Patients are required to have a valid credit card on file with our practice and will be prompted to provide this information as part of the pre-visit confirmation process. A credit card will only be charged for fees associated with missed appointments and late cancellations and payments for non-covered services provided by Array At-Home.
Please notify us at least two days in advance if your schedule changes and you can no longer keep your appointment.
