Your access to Array AtHome services is subject to and conditioned upon your agreement to these Terms, our Privacy Policy, and all applicable laws and regulations governing access to the Array AtHome Systems and services. By agreeing to the following terms and conditions, you agree to these Terms below. Other areas or particular aspects of the services may be subject to and conditioned upon your compliance with additional terms and conditions (e.g. consents, etc.), which will be presented to you prior to your access of those areas or services.
I am requesting access to portions of my health information and the ability to communicate with my Array AtHome health care team concerning my health information via the Internet using an electronic application used by Array AtHome.
I understand that by accessing or using Array AtHome Systems on behalf of another person, I represent that I am authorized to accept these Terms on that person’s behalf and that the person to be responsible to us if myself or the other person violates these Terms.
I understand that the Array AtHome system is intended as a secure online source of confidential medical information. I must have internet access and my computer’s internet browser must be one of the following:
I agree to be responsible for any telephone charges and/or Internet service fees incurred by accessing the Array AtHome Systems.
I understand that Array AtHome System is never to be used as a means of communication to health care providers for urgent or emergency matters. For all emergency matters that I believe might immediately affect my health or well-being, I will dial 911 without delay and/or go to the emergency department of a local hospital.
I understand that my Array AtHome health care team may send me messages via the Array AtHome System. These messages may contain information regarding my health and medical care. It is my responsibility to monitor these messages that are sent to my Account Inbox. I agree not to hold Array AtHome liable for any loss, injury or claims of any kind resulting from messages that I fail to read or act upon in a timely manner.
I have the option of entering a valid and functional email address that may be used to notify me of the presence of messages in my consumer portal. If I opt to provide this email address for notification of the presence of messages, I will update any changes of my email address as needed to provide timely delivery of messages.
I agree that it is my responsibility to select a confidential password, to maintain my password in a secure manner, and to change my password if I believe it may have been compromised in any way. If I share my Array AtHome ID and password with another person, that person may be able to view my health information, and information about someone who has authorized me as an Array AtHome proxy. I agree not to hold Array AtHome or any of its subsidiaries liable for any loss, injury or claims of any kind resulting from my disclosure of the confidential password or confidential information on the Array AtHome system.
I understand that the Array AtHome system contains selected, limited medical information from a patient’s medical record and that the Array AtHome portal does not reflect the complete contents of the medical record.
I understand that my activities within Array AtHome may be tracked by computer audit and that entries I make may become part of the electronic record.
I understand that access to the Array AtHome system is provided by Array AtHome as a convenience and that Array AtHome has the right to deactivate or restrict access to this system at any time for any reason or no reason without notice to me.
I understand that my use of Array AtHome must be appropriate and may not be used to harass, intimidate, defame, or threaten any individual or for any illegal activity and agree to use Array AtHome Systems in accordance with all applicable local, state, and national regulations
I understand that use of the Array AtHome system is voluntary and I am not required to use Array AtHome or to authorize a Array AtHome proxy.
I understand that the Array AtHome system may not be available to me all the time due to system failures, back-up procedures, periodic system maintenance, or other causes beyond the control of Array AtHome. Access is provided on an “as-is, as-available” basis and does not guarantee that I will be able to access the Array AtHome system at any particular time.
The Array AtHome Systems may include text, graphics, images, trademarks, trade names, service marks, logos, and other content therein (collectively, the “Array AtHome Systems Content”) that are owned by or licensed to Array AtHome and are protected under both United States and foreign laws. Except as explicitly stated in these Terms, Array AtHome and our licensors reserve all rights in and to our Array AtHome Systems Content.
I UNDERSTAND THAT ARRAY ATHOME TAKES NO RESPONSIBILITY FOR AND DISCLAIMS ANY AND ALL LIABILITY ARISING FROM ANY SOFTWARE INACCURACIES OR SOFTWARE DEFECTS, COMMUNICATION LINES, VIRTUAL PRIVATE NETWORK, THE INTERNET OR MY INTERNET SERVICE PROVIDER (ISP), ACCESS SYSTEM, COMPUTER HARDWARE OR SOFTWARE, OR ANY OTHER SERVICE OR DEVICE THAT I USE TO ACCESS THE ARRAY ATHOME SYSTEM.
During times when the Array AtHome system is unavailable, other communication methods (e.g., telephone) should be used to access Array AtHome.
We reserve the right to change our Terms at any time. Your continued use of the Array AtHome Systems will constitute your agreement to the changes we have made. The last date these Terms were revised is set forth at the top of this document.
