Need to Cancel or Reschedule?

When you schedule an appointment, you reserve time with a clinician that becomes unavailable to others. We understand that things come up, and plans can change. If you need to cancel or reschedule, please notify us at least two days in advance through your patient MyChart account or by contacting our Care Navigation team at 800.442.8938. 

Cancellations made two or more days before your appointment will not incur a fee.

Appointments canceled with less than two days’ notice will be considered late cancellations, and a fee of up to $100 may apply. If you miss an appointment without providing notice, it will be marked as a no-show, and the same fee may apply. (Note, these fees do not apply to Medicaid and select insurance plans)

Credit Card Required on File

As a fully virtual behavioral health group practice, we require a valid credit card on file for all patients using commercial insurance. Insurance copays will vary based on your specific health plan coverage and benefits. Having health insurance is not a guarantee of payment or coverage for services. Your credit card will only be charged for the following:

Non-covered services

Outstanding balances due to loss or changes in coverage

Non-par providers (i.e., out-of-network providers)

Co-pays, co-insurance, and deductibles after insurance payments

A receipt for any charges will be available under Payment History in your MyChart account. If allowed by your insurance, this notice serves as your consent to being charged for all patient balances.  

If you miss a scheduled appointment without notifying us at least two days in advance, a no-show fee of up to $100 may be charged to the card on file. The same applies to late cancellations made the day before or the day of your appointment.  

Additionally, if Array is not in-network with a federal or state health plan (e.g. Medicare and/or Medicaid), and cannot bill for services, the patient will be responsible for self-pay fees and will be billed according to our rates, which are outlined here:
